Services Provided

PHDM connects families to MEDICAID which provides necessary health care services to FIP/SSI recipients and other low-income persons who meet the state assets and financial or health eligibility criteria. Coverage may include services for maternity, family planning, infant and maternal support, health insurance for HIV/AIDS positive individuals, the blind, nursing home, home-health and physician services, substance abuse services and many other medical services.

Also intake for

• MI Child/Healthy Kids  insurance for eligible children
• Children's Special Health Care Services (CSHCS) assistance for children with qualifying medical conditions

Eligibility:

FIP/SSI beneficiaries and some non-public assistance persons meeting financial and/or medical
eligibility criteria
